when to replace clutch/tranny

How do you know when the clutch is wearing out?
How do you know when the synchronizers in the transmission are going out?

I have occasional trouble with grinding gears while shifting. Clutch can be thrown all the way to the floor so its not that. It happens most often when shifting up or down into 4th, but it has happened on other gears as well. PO said he replaced the clutch about 125K ago. Transmission has never been rebuilt. The grinding only happens maybe once or twice a week on my daily driver, but it is annoying.

Any advice is always greatly appreciated.

Drain the fluid and fill it with some Pennzoil syncromesh, mine shifts much better than it did with the atf that it calls for. You can tell your clutch is slipping when your rpms go up but speed doesn't.
Does the manual fluid still cool the same?

Yes it cools the same.
Previous owner replaced clutch 125k ago so it sounds like a higher milage transmission to begin with? Honestly I would try the fluid but even manual transmissions develop problems. It may be time to consider taking it to a shop if the fluid doesn't help a lot.
