Hey guys, I might go look at a 1990 7.3 Idi truck. The ad says "Engine is running rough, I don't know whats wrong with it, possibly fuel related. Just dont have time to look into it"

I'm thinking it's probably a bad fuel filter, bad fuel or bad injectors.

How would I check to see if it's got a bad cylinder from cavitation or something worse than just the fuel system?

The fuel return caps can get bad o-rings, or the rubber hoses crack and let air in. The nice thing about IDI's is injectors are cheap and easy to change, same with glow plugs. I'd get the GP's off the switch and put a new controller in it. If there's no sign of coolant in the oil I'd just run a few bottles of VC-8 in fresh coolant
