As long as you get the right size adapter to fit, it would work.

I have an early 99 7.3 and i was wondering if a 5" turbo back exhaust will fit.
The outlet from the turbo is 3", and not much more will fit from the turbo down the side of the firewall.

Most people who aren't running a stock downpipe have a 3" downpipe that goes to a 4" or 5" pipe... down the truck or up through for a stack, personal preference.

My dad's e99 powerstroke has a 4in turbo back. That 4in downpipe was a snug fit, so thats as big as will fit; you can go 5in from the downpipe back though....sure will sound good!

Your going to have a HELL of a time trying to make a 4" dp work. It can happen, but takes a LOT of work.

5" exhaust sounds great.

You'll probably get better responses in the sd section of this site. A four inch dp wont fit in an obs truck.
