White smoke and diesel out tailpipe

I have a 2008 f250 what happens is when it warms up it starts to smoke white a lot. Also wet on the tailpipe from diesel and smells like diesel. But not sure what to do about it need help. Thanks!

Clear the codes and see if new ones popup
looks like the pressure sensor is bad

Also short trips in cold weather will cause the DPF to need to regen more often
