That could have been caused by numerous different reasons. Like someone forgot to retorque the u-bolts after that lift was installed. Like the instructions tell you.

In the last picture that is a different rim then the other pictures. Hmmm maybe wrong size tire and rim not equal to the others had something to do with it.
