help with 02 trans

when warm trans shifts hard from 2nd to 3rd and hesitates and hits hard on reverse the other day od light blinked shut truck off and it stoped. speedometer is off by about 5mph any ideas?

no shudder in reverse. i use to have the obd 2 scanner and scope but lost them in fire ill have to have friend run codes for me. fluid not burned. I was a mech for a few years till i got tired of it. i'm more of a drivability and motor mech not alot of trans exp except for r&r. just wondering if anyone here has had this issue and got it resolved.
