severe vibration in steering wheel

I have a 06 4x4 long bed that is stock with 315 bfg's. anyway it has 45,000 miles on it. it has new tires on front, been balanced and aligned. around 45mph it has a god awful vibration like right to left in the steering wheel. if you accelerate thru this zone dont really feel it. but if you drive slow to this point you can get it to do it everytime. dont matter if speeding up or slowing down. and seem to just notice it when going straight down the road. I am looking for any info I am no mechanic but seem like maybe a joint of some kind or bearing going out. where should I start?
