Lack of Power Blue Smoke on 2003 6.0L [NOT EGR VALVE]

Hey guys, hoping someone can help. So far its been to two dealers and a handful of my buddies that are techs.

Problem:
Around 2000-2500 RPM the truck will start missing. Lack of power and blue smoke are very evident. Gas mileage is down to 10.5 mpg. EDIT: This is only an intermittent problem.

Stuff that has been replaced recently:
EGR Valve
ICP
IPR
HPOP

What has been done so far:
The FICM has been checked and is good.
Passed Injector buzz and drop tests.
All vitals appear good (used a mastertech CAN box connected to a laptop)
Recorded a snap-shot of the truck missing, what I see is when the truck acts up the Fuel Level % is all over the chart (from 17%-80%). I know this shouldn't affect performance but when the Fuel Level drops like that so does the ICP.

Anyone have any ideas?

Sorry I just added that it is an intermittent problem. If my PCM was junk I would guess that it would do it all the time. I am thinking that it is an injector or ICP related. When the HPOP went out it sent metal shavings at least as far as the oil filter. My guess is one of the metal shavings made it to an injector and its causing it to stick open but I can not get anything on the scan tool supporting that theory. It passes all the injector related tests the scan tool has and the scan tool will not pick up a misfire, not even in mode 6 data or the snap-shot that I took while it was missing. The other thing I am thinking it may be is a bad connection at the ICP. There used to be a programmer on the truck (came with it when I bought it) but I removed it and hooked the factory harness back up because where it spliced in at the ICP sensor the splice got oil soaked and melted (old ICP was leaking). When I did this the problem got much better but still evident.

Could you have relatively large peice of debris in an oil pasage that feeds to your injecters that may be movin around and causing intermitant blockage? Good luck with your hunt.

Thats very possible, I'm hoping to find someone who has already been through this lol... but I think I'm on a wild goose chase
