Serious/Annoying Vibration

Hey guys here's the scoop, I have a 2000 f350 with 199,500 miles on her. She has a 4.5" Icon hanger kit lift(approx. 3 years old) and new 285/75/16 Toyo Open Country AT tires as of yesterday and while having the tires put on I had them check alignment also. I replaced all three u joints and carrier bearing approx. one month ago. While driving I am getting a pretty good vibration and i'm not sure where it is coming from, once I hit about 45 mph it starts and around 65-75 it is almost unbearable. It seem while i'm at highway speeds it seems to kinda go away when I let completely off the pedal. What is your guys take?

When did it start? Do you feel it more in your seat or your steering wheel?

It has been this way for a little while now not really sure when it started. I feel it more in the seat, I noticed the steering wheel feels pretty smooth.

How high did you lift the rear? Did you shim the carrier bearing? Have you ever pulled the slip yoke apart?

The drive shaft has never been taken apart so it should still be phased correctly. Today I decided to pull the rear driveshaft and throw it in 4x4 and take her for a drive and see what would happen, I found that there was no vibration at all now from 0-90mph. What are the best u joints for super dutys and how much should the carrier bearing be shimmed with this setup. I never did shim the carrier bearing.

What's your arrangement? Sclb or scsb. I'd use spicer, Moog or oem for U-joints. My guess is 1"-1.5" of spacer but it all depends on your arrangement.

1.25". If you get a small vibration on take off add an 1/4". Also replace your carrier bearing. If you don't feel like messing with the u joints and carrier bearing take if to a drive line shop and have them replace everything and do a balance.

How often does a driveline need to be balanced or only when you're having issues? Where did you come up with a 1.25", just wondering for future reference.
