2008 f350 with 180k.
at idle truck is quiet, reving motor truck is quiet
at takeoff from a stop there is a whistle noise sort of squeal noise.
what the hell could this be? tensioner? turbo? any thoughts and how to check?

sounds to me like your hearing turbo spool.. turbo diesels definitely whistle .. Is it a whistle or a screech/squeal.. screeching/squealing isn't normal... maybe try recording a video of it and posting it here
But if it was that you would constantly hear the problem regardless if it's in drive or not... OP stated that the engine doesn't make the noise if it's in park idling or revving it in park. Don't think that's the problem
Maybe some sort of exhaust leak under load? Try to put a little load on motor in drive and see if it happens when turbo spools up.
