What is the proper name for this?

So I am about to begin buying all of the parts needed to add the factory backup sensors to my 06 cc lariat. I have found pretty much everything needed through either Tousley or eBay. The only thing I am having difficulty finding is the bezel that has the hole for the 4x4 knob, the power back glass switch, AND the back up sensor button. Mine (because I don't have the sensors) only has 4x4 knob and power back glass switch. I posted a picture of what I need below.

What is the proper name for this, where can I get it, and how much does it cost??
