injector issues 2,3,4,5

2 months ago out of the blue my truck started starting hard and running very rough until completely warm, once warm the truck continues to have a slight miss when driving. i brought the truck to the dealer where i bought it from used 6 months prior, i get a call later in the day that injectors 2 and 3 are bad, they went good on the parts I had to pay labor.
fast forward 1 month to the day and the truck went right back to the same issue, made the appointment, got the call that 4 and 5 are now bad. now im on the hook for parts and labor. 1000 in change later the truck runs great for 1 day. I went out to the truck this afternoon no more hard start but roughs rough and has a slight miss once warm.

I did some home work on here and did some checks
my ficm main power is 48.5 volts constant
battery volts dont drop below 11v cranking
my ficm logic power is 11.5 cranking 14.5 running after the GP has turned off
fuel pressure was checked yesterday at the dealer and is above 60psi, i also have the blue spring
fuel filters done 2500 ago
running rotella t6 syn, no additives done 2500 ago
diesel kleen w/ cetane boost every tank

I cannot continue throwing a 1000 bucks into this truck every month I have no problem fixing what the issue is but 2 injectors a month is getting old, any and all help is greatly appreciated

A few things to try/check.

Monitor your ICP Pressure vs ICP Desired. They should pretty much match, being around 600-700 at idle, and spiking under load, but they should always be hand-in-hand. If they don't, could be an IPR or HPOP issue, or some sort of HP leak in the system.

Add Rev-X, see if it helps/fixes. There might be some gunk or varnishing inside the injectors that was put there by some past oil or junk, that needs cleaning out. It won't hurt, and it might help, and it's less than $100.
