sucking air?

My BIL has an 04 6.0, he has worked it hard and is now having some issues. Says it is hard to start cold, runs fine once warmed up. Supposedly was told he had 2 bad injectors, but he has big truck experience and feels that it has to be sucking air someplace. R&Rd fuel filters, still the same. Had his FICM rebuilt has +50v on it.

So he needs to have some direction to go for more ideas of what to look at. Could it be ICP related, a leaking hpop? I asked if he had changed the Orings on the filter caps and he said no. Also he said he can feel/hear the frame mounted fuel pump and it doesnt seem to shut off like it used to when he tries to crank/start.

Truck has around 130000 miles (hard ones on it). Any ideas appreciated.
