First diesel

Just bought a 06 E350 with 13K miles, was used by a local hospital for transporting staff. It probably has 13 thousand hard miles and no mantainance, but I couldn't pass it up. I use it in business towing a 7000 pound trailer about 35000 miles a year. Other than routine maintainance is ther anything I should keep an eye on? When this thing is down, I'm out of work, don't want to take chances.

I noticed there is shiney black 'orange peal' in the tail pipe. Is this a cause for concern?

Any help would be appreciated.
