'04 6.0 powerstroke cold starting

I have an 04 F250 with the 6.0 PS and it recently has become very sluggish on cold starts and runs like crap for about the first 2 miles down the road. It has 206,000 miles on her. I live in the deep south so its hardly ever below about 20deg outside. However, the colder the temp the harder it is starting and running. When I try to crank it in the morning it will fire and go dead 5 or 6 times before it will finally start and keep running and then it sounds like its running on 4 cylinders. It will also blow black smoke out the exhaust during that time. After it warms up (approx. 2 miles down the road) it starts running smooth and the smoke goes away. Does this sound like the FICM and/or injectors?
