03 6.0L runs very rough

my 03 F350 started blowing black smoke the other day, and now its running quite rough from idle till around 2000 RPM (which is when the smoke starts) sounds like its missing on 3 or more cylinders at idle especially after it warms up. I took my EGR out and cleaned it (although it wasnt that dirty) that made no difference. i just finished testing the FICM and this is what I got,

Key ON engine OFF 48v
Key ON engine ON 42v at the start then it slowly climbs to 46. some odd volts after about 20 seconds and stays there, i thought i read somewhere that the spec was 48-45v but im no expert.

any help would be great!

Before you start looking at the ficm voltage make sure your battery voltage and batteries are in good shape. Any codes or lights up on the dash?
