Won't rev over 2200 rpm's

Just traveled 1700 miles without a problem. Until the next day. Hard start @ first when cold. Then it smoked really bad if got into the fuel a little harder. The next day it wouldn't start @ all. Thought I had water in fuel. Drained the tank, pulled sending unit, replaced both filters and added new fuel. Would crank and try to start but no success. Unplugged icp and still no start. Hooked my service truck to the batteries and when it got to 13.2 volts it started. But now it won't rev over 2200 rpm's. When I hold it to the floor it reaches 2200 rpm's and it smokes bluish white really bad. Any clue? My everyday driver too!

Guess I should have added that the truck is a 2003 f-350, with a 6.0, torque shift. It has an egr delete, cat removed, no other mods or programmer/monitor.

Having a Similar issue whenever I have less than 1/2 a tank of fuel.... Losing my damned mind!

check the air filter, try revving it up with air filter not installed. other than that I would say it is definitely fuel related, If it is not "starving" for air, then the fuel alone is what will regulate RPM. Not sure how the governor works, maybe that also has something to do with it, but I am not sure of how to test it. Also usually a bluish colored smoke is an indication of burning oil.
