Another problematic 6.0!! Please help!

This truck is my daily driver and I need to fix it ASAP! Thanks in advance for any input.

I'll start by explaining my issue and follow with a list of recent repairs that were done in efforts to chase the problem.

I go to start it after sitting for at least a couple hours, it will crank and start and immediately die. I have to press the throttle a little bit and crank it in order to get it to start and idle. It will idle (smoothly) as long as I leave it sit there. I jump in to leave, as soon as I put it in gear (R or D) it dies just like I turned the key off, no sputtering etc. I have to drive it with 2 feet (gas and brake) until I'm moving and for the first 2-3 stop lights. After that it idles fine and runs perfect. It is not lacking power at any time during this process, just simply won't idle when I first start out.

DTC codes:
-EGR flow (due to EGR delete kit)
-Occasional Cam sensor
-Occasional P0611 (FICM)

Recent changes:
-Updated STC fitting
-Replaced IPR valve with a known working unit and a new updated screen/o-rings. (screen was punched on mine causing a no start issue, this problem has been ever since re-assembly.)
-FICM wiring harness
-EGR delete
-Replaced ICP sensor with good known working unit.
-Replaced throttle pedal with good known working unit.
-Replaced FICM with good known working unit.

What next??????

I don't think its a HPO leak since it runs perfect when warm and oil is thin. I can let it come all the way up to temperature in and it still does this.

Furthermore, I will not take the truck to a Stealership and have them charge me $200+ for diagnosis. I will figure this out on my own and the help of you guys!!

I have an AutoEnginuity (AE) scanner I bought and have played with a little but can't figure anything out with it...

Any help is better than nothing!
