94 E350 Dually Tranny problems...

Have a 94 non-turbo E350 dually. The tranny will only shift manualy and it only has 2nd and 3rd. If you start the Truck and throw it in drive it will start right off in drive, you need to drop it into 1st or 2nd but even when you drop it into 1st it will start of in 2nd. The tranny had problems before this. You would be driving down the road accelerating from a stop and once in a while it would rev right up and the OD light would be flashing. What could possibly be wrong?

Could also be the throttle position sensor on the injection pump. If it is *way* out of wack, it will think that you are already at WOT, and will not shift into a lower gear. I had an ambulance with an intermittent O/D light flashing and horrible shift problems, and it turned out to be a loose TPS sensor plug.
