Wiring up Baldor motor? (pic)

got my motor for my centrifuge and I'm having a little trouble figuring how to wire up the switch and extension cord...

I have the wires put together as per the instructions for 115v application, but I'm not sure which wires to hook to which wires on the cord. I looked at the schematic, but that didn't help me much.

Any help would be great!

It doesn't matter with 115v, but I would go WH to WH, and BLK to Y

Does the ground from the plug to to the green lug and then the blue/orange just get taped?

Can you take a pic of the wiring diagram? Yes, the GRD will be fastened to the ground lug in that motor box

There's no diagram on the motor itself, but I found this online. It's at the very bottom of the document
