fuel surge, bad mpg, white smoke, rough idle at start?

ok guys here is a list of problems i am having what could it be? when i first start my truck up it idles seriously bad like a dragster, big cloud of white smoke, after warm up driving or cold drive down the road the truck surges forward while maintaining throttle (doesn't happen all the time but), u take off at a light easy and it lunges forward like you gave it a descent amout of throttle (again doesn't happen all the time but), it's getting 9mpg, and i just bought a tony wild man chip for it and when you switch positions the truck almost completely dies, my brothers truck doesn't do that. and i also notice that it idles alot lower on all the other settings than the stock setting but the stock setting still seems to idle a little lower to. i know that could just be the chip but all the other problems i listed before i mentioned the chip are happening with or without the chip, i just mentioned the almost stalling just in case it might help in finding the issue.
