Is my clutch going out...?

It seems like in the last week anything that could go wrong has gone wrong with my truck, money money money.. So lets just keep it up. When i push in my clutch and barely throttle my truck i can hear a clicking in the clutch. I know its the clutch because i can feel it through the clutch also. I've been told that means my clutch is about to go??

Also (and this may be because of the clutch), when im driving down the road at about 40 mph in 5th gear, low rpm, my truck feels like its jerking itself forward? What could that be?
