Take the boot from the hole in the door jamb, then you can somewhat see inside the truck. From there, either use a ziptie, coat hanger, fish tape, etc and put it thru the door jamb then get up under the dash and see where it's coming from. On the drivers' side it comes out up above the ECM if I remember correctly. Then it should be easier from there.
If it's still too hard to get from there, take the ECM out.
As far as getting it thru the boot, take it one step at a time. Remove the boot's grommets from the door and door jamb then scrunch the boot together so it's short and fat. You should then be able to get the wire thru the boot.
Then remove the door speaker, put the wire thru the hole in the door where all the wires are and grab it thru the speaker hole.
