Truck surges when at low rpm's

2002 F350 w/ 177k miles

This seems to happen randomly to be honest. Doesn't matter if the truck is coming off a cold start or is fully warmed up. I notice sometimes when I am "coasting" around 40-50mph and very low rpm like 1300ish, if I am just barely touching the gas to maintain a constant speed say if I was going up a very slight incline, the truck will surge in and out. Almost as if it is shifting in and out of high gear. if I let off the gas completely it coasts just fine right down to an idle. If I hit the gas more, the rps will shoot up and it will generally just take off without shifting at all.

Does anyone else notice a problem like this? could this be injector related or trans? I just had a very good mechanic look it over and he told me the #5 injector had signs of it going but said I should be fine running it as is instead of replacing it. He also said he thought it shifts just fine and showed no signs of any trans problems.

IDK if this is just me being very finicky and picky with my truck and how it rides, but it definitely seems to slam/surge hard if I am at a low rpm and coasting around 45mph. If I tap the pedal quickly it seems to slam/surge hard.
