99 F350 chugging at take

Question? I have a 99 F350 6speed srw 4x4 with 301k miles. When starting out in first the truck begins to stutter,like your not pushing in the clutch when slowing down. I'll push in clutch and shift into 2nd, no problems after that only at stops when taking off. no stutter while going through the other gears. my question which would be the most likely culprit? thanks for y'alls input.

couple of theories (in no particular order) these would be evident reversing from stop as well as starting from stop in second gear
1. glazed hardspot on flywheel causes grip/slip so as you are engaging the clutch the truck shudders
2. broken motor mount tilts motor/trans at take off putting drive line in bind (open hood, set parking brake, let clutch out slow and see how far the engine rocks)
3. throttle position sensor flaky right off the line so your partial throttle at takeoff isn't registered and you are taking off at idle and bogging the engine (sensor bolted to pedal with two t27 or t25 torx bolts, there will be a round hole with a slot in it, twist it with a slotted screwdriver, it is lightly spring loaded and should spin backwards when you relax your grip on the screwdriver. if it doesn't turn back easily squirt some blaster in it and work it a bit till it frees up and put it back on.)
4. how old is the fuel filter?

'99 f-350, 6sp, white,
extended cab short bed
bone stock 2wd + lift gate
