The trans is out and it's definately an exploded pressure plate. I didn't personally see it but the carnage sounds intense according to the shop doing the replacement.

As far as I know, there's no adjustment on hydraulic clutches. Sometimes the pushrods have adjustable length to set the pedal height and throw but not on my truck. I could be wrong here but I think I read the pre-99's had cable operated clutches.
