1999 f650 stumbles and smokes???

I have a 1999 f-650 service truck. it starts fine hot or cold, it idles fine no skip shake ect, if you stand on it it stumbles and blows a whiteish blueish smoke stream out and reaches 2740 rpms and will not go any higher. If you raise rpms slowly from idle on up slowly no skip no smoke all seems ok yet still will not rev over 2740 rpms. Driving it sorta does the same thing-cruises smooth but still will not rev over 2740 rpms. New filter, pre screen clean, good fuel pressure ect ?? Any help please contact me, 1-774-406-7289 Richie

Tried to start it this morning and would hardly run, cylenders 2 and 4 on d side and 5 and 7 on pass side stayed cold and after running for a bit some began to run but never got as hot as the obviuosly good running cylenders, once warm same deal as above.
