So...Do YOU think its dusted?

'99 (early) PowerStroke, 192k, 2X4, CC, SB, Stock (save some gauges)

Started with a miss and poor driveability on cold start. Has moved to a dead miss on start with a miss at warm idle...still runs reasonably well warm.
Sometimes runs perfectly fine.

Oil Cap test - I get slight white puffing.....but it won't "blow" the oil cap off when the engine is warm (or cold for that matter). It does come off....although it is difficult to tell if it is from pressure or vibrating off....

Changed oil/filter, fuel filter (pretty dirty), CPS. Checked IPR, HPOP level...all fine. Not much change.

In Dec: Buzz test SAT. Contribution intermittent low 3/8 (thanks Gray CPS)

Knowing I have a dead cylinder, I took it to the dealer:
Buzz test SAT, low con on #5 (P0275 code), and states that Crankcase Pressure excessive (assuming blowby excessive).

Dealer states he believes primary cause is dead hole due to rings.....recommends long block replacement. Says it might have faulty injector but thinks its a compression issue based on "the high mileage of the vehicle"

Personally, I think dead injector. I'm not seeing enough puff from my filler neck to think it serious blowby....and I'm not getting any white smoke through my exhaust (actually not any smoke...barely a blue puff on start...bad injector o-rings maybe?)

Insult to injury....not fifteen minutes from when I told them I would pick up the truck (ie You are not servicing my truck), they called back telling me my power steering line ruptured....145.00 replacement.

So....what do YOU all think....am I nuts?
