1997 Powerstroke no start/hard start

First of all this truck is completely stock and just had the cam position sensor replaced, mechanical fuel pump, and IPR. Truck will not start right away. after about 10 tries to start and peddling it, it will start and only stay running at higher rpm. after a while of running (not driving) it will shut off. It is misfiring and has the following codes 1211, 1212, 0344
Any ideas?? Your help is greatly appreciated.

I havent checked but if I remember the 12 codes are ICP and the 344 is a cam code.
