1999 f350 problem

Yesterday it started running rough so I went home it blows white smoke today I took the oil cap off and it will blow the cap off

What do I need to have done

What would make it crack or blow a hole in it?
It ran fine I stopped to fill it up and it ran like crap

To find out which injector , get your self an infrared non contact thermometer.
Non-Contact Infrared Thermometer with Laser Targeting
By checking each exhaust runner , you should be able see which cylinder by the lower temp reading.
