Rough running warm, white Smoke cold. Help!

Hey guys, new here and to the powerstroke world, looks like ive come to the right place. I've got a 99 6 speed with 260k miles. Ran great until recently and has gotten progressively worse.
Symptoms: when warming up i see intermittent puffs of white smoke. Seems to be about one per second or so and goes away once it's warmed up.
Runs ok until oil temp reaches about 195-200 then runs awful but only below 1500 rpms. Has low power, shakes, and seems to "chug along". Once I reach top gear and get the rpms over 1500 for more than a few seconds it will clear up and run almost normal. Also seems to clear up faster if I give it heavy throttle.

Pulled the valve covers and checked the harnesses, looks ok. Noticed one injector is different from the rest and stamped "ITS" on the cap. Fired it up and they all seem to be spitting about the same Amount of oil but the ITS one is also spitting a small amount of oil from what appears to be the side of the top yellow cap/cover. It's the front injector on the driver side and it was spitting the oil out far enough to get All over the alternator and top of the motor. Wasn't able to get the oil temp up enough for it to start running rough without driving it but even when cold and moderately warm it seems to run a little different than usual and sounds kinda rough.
Checked for a cold cylinder with a infrared thermometer before pulling the valve covers but they all seemed consistent. Also, even when warm enough to show symptoms it doesn't seem to idle any different, mainly only between 900 to 1500 rpms.
I would imagine its just time for an injector rebuild but don't want to drop a grand or so and still have the problems.
Has a good Amount of blow-by, but not enough to blow the oil fill cap off when upside down on the filler neck. And doesn't seem to have anymore than before.
Will get a video tomorrow to give you guys a better idea of what I'm seeing.

Yep sounds like injectors. Get it hooked to a scanner and run some tests.
