Clatter consistent with engine rpm

Hey guys. New to the site, and new to the powerstroke family. Recently purchased a 99 with 240k on it. Seems to run ok but feels a little sluggish to me. Has a pretty good clatter to the engine that's consistent with the engine rpm? I've been digging through threads for awhile now. Anyone help me? Give me some direction to go in?
I'm a truck driver by trade and thought maybe it needed what we call a "overhead" or valve adjustment. But from my research online it seems that's never really heard of on powerstrokes. I've seen a lot of right ups about injectors(especially number 8) and fuel pumps not supplying enough fuel. Seems to have a VERY mild skip at a idle also.
One more question. How do I tell which tranny I have? The 4r100 or whatever or the other one. Still learning my powerstrokes so bear with me. They seem a little more complicated with there high pressure oil systems but just as dependable as the rest. Any help would be greatly appreciated and this seems to be a great sight.

I should add. I'm not talking about the normal diesel clatter. This is a distinct mechanical clatter that isn't normal operation. I've been around enough powerstrokes to know how they sound but this is different.
