hole in piston!

I have a 2001 f-350 powerstroke with 347,000 miles and it wasn't running right and was blowing allot of blue/ white smoke. I Did a cylinder contribution test with A&E and came up that the problem was on cylinder 2. I pulled the motor to find out what was wrong and after pulling the head a found i had a hole in the piston. my question is what would cause this? i had gauges and it never went above 1100 pre turbo. The previous owner said he put stage 1 injectors in but as no proof. Do you think it might of had an injector issue? all the other pistons look great.
