Surging?

Well I was driving to town today and I seemed to notice a few things. One was that I cant hear my motor until it gets to around 40 and then it seems like it fires back up. The other problem seemed like it gets to much fuel and seems like it surges but rpms stay the same and so does the speed. Its a 2000 F350 all Stock. 249k miles fuel and oil need changed bad which ill do this weekend. Air filter looks pretty good I'm not sure of problems and since I got this truck about a month ago. No codes pulled either
