oil cooler issues

i made mistake tonight, my oil cooler has been leaking oil on the ground, so tonight i decided to fix it. i got new o-rings and gaskets, put it back together with new oil and coolant. it ran for about 5 minutes and died then oil gurgled out of the coolant reservoir. i think i got the oil cooler together wrong because i was in a hurry, or maybe i cut an o-ring. but my big issue is did i screw any thing up? do i need to flush the system or just drain it? why did it die?
