Water in Oil....

Hx: '02 E-350 w/ 307,00. I found the crankcase full of water/oil milkshake. I have replaced the oil cooler w/ new o-rings and flange gaskets TWICE. Thinking that I may have pinched an o-ring. So, 45 qts oil, 15 gal. coolant, 3 filters later i still have water getting in the oil.
What the hex2L is going on? Where is the water getting to the oil at? Engine runs perfect not a miss even when it was running on the slurry, good power. I'm pretty good at turning a wrench so any help is appreciated.
