have you checked the simple stuff?, tire pressure, dragging brakes, ect
dose it seem to run well or dose it lack power?
Do you beat on it or are you easy on the fuel pedal?

I'll start with the simple checks.

How long have you owned the truck, is there a change the previous owner swapped gears?

Last time the Air Filter was cleaned/replaced?
Last time the fuel filters were changed?
Last oil change, and tranny fluids were changed?

It just few things that wouldn't take too long to check. After that we'd need to know more about the truck's configuration, and your driving style.
