Vibes at 40 mph

I have been trying to chase down a pretty good vibration at 40mph. I initially attributed this to my tires, they were well worn, and needed to be replaced.
I put some new Dura Grapplers on it, and still have the vibration. The vibration feels almost like wheel hop. The shop that did the tires is very good, and I have never had problems with their balancing job in the past.
I have checked to make sure that my front hubs are disengaging, The only other thing that I can think is maybe my rear drive line is out of balance? My truck is a 01 extended cab long bed with a carrier bearing.

Any ideas?

Is it lifted? If so has the carrier bearing been shimmed correctly? I would get it off the ground and get things spinning and see if you can track it down.

The truck has 208k on the clock. It is leveled in the front, and has an ad-a-leaf in the rear. It has been this way for the entire time I've owned it (2 years), and the vibes did not start until approx 3k miles ago. I may have to take it to a DL shop to see if they notice anything wrong.

Time to get it up on a lift and it should be obvious where it is coming from.
