South bend clutch issues

Ok so i go to stop at a stop sign and i put the clutch in and it works good. All of a sudden something goes thunk (not loud but i heard it) and the clutch lets go and it starts driving again. Hit the brakes and stalled it out so i wouldn't hit anyone! Now if i start it with the clutch in, it stays put but as soon as you let the clutch out you can't stop in gear anymore until you shut it off and start it with the clutch in. I just put the clutch back in because somehow the disc closest to the flywheel managed to pop off and kind of messed up the teeth on the shaft and on the disc. Could this be the problem again? When i drive it and pin it all the way there is no slipping what so ever but you would think that if the disc popped off the shaft then it would slip no? Can it even pop off the shaft!
