Poor tuning, bad injectors, other mechanical problems, immature drivers.

On the passenger side valve cover there is a sticker with light blue writing, informs you it has 30 more hp, and conforms to California emissions, another way is the glow plug relay will have a "shunt" in the middle of it, and if you remove the valve covers, the injectors are ab code, also afaik the PCM in my California model is mme2, so auto and 4x4 with split shot programming

Valve covers can be swapped or painted over, the late 96 CA emissions trucks don't have a shunt, pulling the valve covers and/or PCM is a waste of time.
The EASIEST way to tell, by far, is if you look under the turbo there will be a round black disc (looks similar to a hockey puck) right behind the fuel pump. If your truck has this part, you have CA emissions. If not, you don't.
AB injectors on stock tuning will not smoke unless there are other things wrong with the truck.

Ab injectors on a non Cali model will run poorly without tuning as well
