personally i wouldnt make a hitch, i have one now, and we've modified it atleast 6 times now for being on different trucks at the farm. and iv priced a new hitch. for what we spent modifying i could have bought 2 factory hitches which are alot cleaner setups. that would of worked on all 6 trucks we had lol

just my $.02 my next hitch will be bought not made.

but if you would like i can try and snap some pictures tomorrow of my hitch on my truck

I made mine out of 5/16 plate and a 2 5/16 ball. The plate is bolted into the factory bed bolts in the back, and ubolted to the frame in the front. Cost me 50 bucks. I've pulled up to 30K with it no problem.

I'll snap some mount pictures later.
