jumping RPM's

Sometimes when starting truck (more so when its cold outside) it starts up and jumps from 500 to 1000 RPM's within a second. It only stops if i keep it constanly revved at about 1000 for a few seconds. Whats possible cause/fix?

these trucks are designed to idle high when they're cold

let me re phrase... it repeadetly jumps until i push and hold the accelerator, not start low and then jump and stay... it has done this for years but i have just lived with it...
