Help Me Identify This Clutch, Please!!!!

Long story short, When my dad gave me my truck he wanted it to have a fresh clutch. He had never had it replaced and it never gave him any trouble. We took it to a south bend distributor and had a South Bend CON OFE installed and 18,000 miles later I need a new clutch. Come to find out I wasn't sold a South Bend and have no idea what I got. Pretty P.O.ed about the whole thing right now. For all I know he could of sold me a used clutch and charged dad for a new clutch. The guy is no longer listed as a distributor btw
