Super hard shifting?

So today I got in my truck and started driving it and when I would take off it would rev out to 3000 rpms then shift into 2nd really hard, and when it was in second it would kinda hesitate and the rest of the gears were fine. What could it be????? Also it's a 95 4x4 with 176k on it.

was your OD off light flashing on the shifter? do you have and mods done to the truck?
