Fuel issues- No codes-HELP

This may get a little lengthy, but bear with me please. I've got a 95 F250 w/ 200K+ and no major issues to date. A few days ago I left home with the fuel gauge showing about an 1/8th and before I went a few miles, the gauge fell to empty. No big deal. It's an old truck and sometimes the gauge gets a little squirrelly. Unfortunately, before I went two miles, it started running as if it were running out of fuel. It did this for several miles then died. I called for help, added more fuel, but still no start. I towed it home and added even more fuel. I switched tanks and after a few minutes, it fired and ran great for maybe 20 miles. Then without warning, the whole situation started over AFTER it got a new tank of fresh fuel.

I had more diesel brought to me and this time filled the fuel filter housing. It fired up and ran great until the housing got empty then died. No start. Towed it home and scanned it with my MT2500. No codes were present and I cleared old codes and tried again. Still no codes. Obviously it was a fuel issue, so I bought a fuel pump and filter. Got it all back together today and got the exact same result. It will start when I fill up the filter housing and run great until it's empty, then dead.

I assume the issue is between the tank and the fuel pump. Any suggestions?
