Early 95 PSD Hard Start, Rough Idle, Runs Rough, Dies When Warm

A quick overview of the truck.

Early 95, 4x4 Manual with 185k miles

The truck will start after several cranks and usually have to lightly tap the accelerator before it fires. Once it's started it has a rough idle that obviously 1 or more cylinders are not firing. Eventually the truck will idle on it's own without having to keep it around 750 RPM. After running for about 10 minutes it shuts off like I turned the key off. After sitting for 30 min to an hour, it will fire back up, still running badly. No black, blue or white smoke while running. After several starts or if it runs for at least a minute, it starts again much easier than the first time. But never runs smoothly, always missing.

What's been replaced
-IPR (can't guarantee it has the edge filter like it should)
-CPS (multiples)
-IDM
-HPOP
-fuel filter
-LPOP
-fuel pump
-pigtail to UVCH driver side
-checked & unplugged ICP sensor
-no CEL's
-have not put it on AE to check contribution or do a buzz test

I'm at a guess that it is either O-rings or injectors. Anybody had similar issues?

Have you tried pulling the injector harnesses one by one while the truck is running to see if when you unplug one it makes a difference? If you unplug one and it doesn't change the running state of the motor, you've then ruled out which UVCH is bad.

Have you taken off the valve covers and inspected the harness feeding the injectors/glowplugs? When these fail, they usually burn up inside of the valve cover.

I have not pulled the covers and looked at the harnesses.

I have also not unplugged each injector individually.

I was aware that someone had recently swapped out each with a know good harness and had no effect.

BTW, did not mention this is not my rig. But new info I have!!

In asking more questions, there was a piece of metal (like a staple) jammed in the LPOP and why it got replaced (just after the HPOP). Supposedly the HPOP was replaced just before the truck started stalling and the LPOP right after. My hunch is what went through and got stuck in the LPOP is now either in the HPOP or made it's way through and caused damage regardless thus making another HPOP in order
