1996 powerstroke is studdering during acc.

I have a 1995 F250, 2wd, Manuel trans. 7.3 powerstroke that has about 175000 miles. This is what is happening, a couple months back my truck was having a probblem while running cold (PROBLEM: it would start with no issue however when you put in gear and started driving the truck would make it about a minute up the street and the start studdering as if it was not getting enough fuel. During the studdering what smoke would pour out the exhaust until it wormed up then would drive fine. I went a head and replaced the fuel filter and air cleaner. The truck continued to do this for a couple months which continued to get worse. As I was driving the truck today it was falling on its face even after it wormed up. I can't get it to accelerate quick at times and others it will be okay. And some times I will get continues studdering with white smoke from 1500 rpms to 2500 rpms; I am thinking that is the injectors
