Fuel Leak

On a 96 ford f250 powerstroke in between the turbo housing and the fuel filter is a black bosch round cap with a bolt under it, that goes into the fuel rail. truck poured out all of the fuel while driving pulled over and the bolt completly backed itself out tightned it and put fuel in it drove it 5 miles and the thing backed itself out again....any thoughts?
