03' F350 diesel surging when cold 7.3

The F350 I have surges some times on cold mornings when I start it. Only slightly. I have been outside the truck when it warms up and all off a sudden the idle will go higher and it sounds like a hurricane comming out of the exhaust and then it will kick down to a lower idle. What is happening there?
