please help whats wrong with my transmission :(

e4od automatic
just under 198k
was holding out first.. lift off gas would go into 2nd..
now it doesnt want to shift to 2nd.. holds out.. sometimes holds 2nd into 3rd..
just revs doesnt shift..
i just got home.. had no reverse..
pissed slammed the gas it grabbed reverse at 3000rpms or so

transmission shot?
another idea?

please help...
